Transportation Release Form

Mandatory Transportation Release Form for Kindergarten, ECE, ECEAP, Transitional Kindergarten, and Special Education students receiving special transportation- see form below

A Transportation Release Form is required and must be signed by the parent before the student's first day riding the school bus. Students must always be released to their parent/guardian or authorized person. You may provide up to 6 authorized people on the form. Proof of identification will be required in order for the student to be released from the bus. Transportation personnel are dedicated to making students' daily bus ride a safe and enjoyable experience. Completed forms can be submitted:

• Mail: 805 2nd AVE SE, Issaquah WA 98027, Attn: Transportation Release Form
• Kindergartners- bring it with you to the Kindergarten Ride-along
• Last resort, you make give it directly to your bus driver

It is very important that we receive the release form before your student's first day riding the school bus. 

If you need to add a person to the release form, you will need to fill out a new form and send it into the Transportation office or hand it to your bus driver.

Please attach or enclose a recent picture (headshot only) of your student. You may only have one face to remember, but our drivers have many faces and names to learn and remember.
